For any insurance company not listed, I am considered an "out-of-network" provider. I am happy to provide documentation (“Super-Bills”) that you can submit to your insurance to get a portion of my fee reimbursed. Please contact your health insurance and ask about your out-of-network benefits.

1) What are my out-of-network benefits for mental/behavioral health?

2) What are my restrictions (if any) to seeing an out-of-network provider

3) How much will I be reimbursed for the following code(s) 90791, 90834 & 90837. *What is not reimbursed by insurance is your out-of-pocket responsibility.
